Before each appearance (regardless of what the purpose of the appearance is) you must confirm court dates by filing a Form 14C Conformation with the court .

• If your case is in the Ontario Court of Justice, your case may be heard last if you fail to file this form.
• If your case is in the Superior Court of Justice, your case may NOT heard at all if you fail to file this form.

You can file it in person or fax it to the court:

The form must be in the court’s hands before 2:00 p.m. two days before the court date.
The purpose of this form is to inform the judge of:

• whether you discussed the appearance and the issues with the other party
• whether the parties expect the matter to proceed (or are seeking an adjournment)
• what issues the parties need to deal with on the appearance
• what documents filed with the court the judge should read
• how much time do you expect to need of the judge's time for you side of the case on that day
  (if the total estimated for all parties is more than one hour, you may be required to obtain a special long appearance date)
